Urban Recovery is committed to providing care and treatment for individuals dealing with substance and alcohol use disorders. Our medical facility offers comprehensive services, including detoxification, medication-assisted treatment, rehabilitation, and recovery therapy. We are currently looking for Full-Time and Part-Time Health Services Coordinator (LPN) to join our dedicated team!

As LPN, you will provide personalized nursing care based on individual client needs, applying recognized nursing techniques and established standards under the guidance of a registered nurse. This role offers the opportunity to work alongside other dedicated professionals while expanding your clinical knowledge!

Available Shifts:

• Day Shift: 7:00 AM - 7:30 PM
• Night Shift: 7:00 PM - 7:30 AM

Essential Duties:

• Administer medications promptly and according to physician orders, following Safe Nursing Practice standards.
• Educate patients about their medications and document their responses, understanding, and adherence in the medical records; notify the registered nurse or physician of any abnormalities.
• Log all patient activities, such as admissions, discharges, transfers, and emergency room visits, and report any irregularities to the registered nurse or physician.
• Track the progress of the patient's detoxification, documenting behavioral responses and alerting the registered nurse or physician to any issues.
• Document treatment interventions and patient responses; report any abnormalities to the registered nurse or physician.
• Communicate with the treatment team regarding patient progress and notify the registered nurse or physician of any concerns.
• Perform withdrawal assessments and monitor vital signs as dictated by the treatment plan.
• Gather data for Continuous Quality Improvement (CQI) initiatives, including census, Kardex, risk management, incident reports, infection control, and medication event reporting; inform the registered nurse or physician of any irregularities.

Minimum Qualifications:

• Graduate from an accredited nursing program.
• Current NY State Board of Nursing License and Registration.
• Current BLS/CPR certification.
• At least two (2) years of nursing experience.
• Preferred experience in addiction treatment.
