As a unit counselor, you will provide critical support to patients by eliminating barriers to treatment and ensuring continuous involvement in addiction recovery programs. You'll offer guidance, teach coping strategies, and inspire patients to remain hopeful as they work through their recovery process.
Core Responsibilities:
Manage and maintain assigned caseloads.
Facilitate weekly one-on-one and group counseling sessions as assigned.
Conduct clinical assessments and intakes as necessary.
Maintain professional, empathetic, and courteous communication with patients at all times.
Collaborate with counselors and nursing staff to ensure patient care is seamless.
Encourage positive communication between staff and patients.
Ensure patients follow the program's daily activity schedule.
Educate patients on daily living skills to promote healthier routines.
Monitor patient behavior for inconsistencies and report findings to appropriate staff and shift reports.
Support monthly skills workshops related to chemical dependency under a master-level QHP.
Assist in processing and distributing incident reports according to agency-wide guidelines.
Help prepare for weekly case conferences and patient case reviews.
Assist with the discharge process, ensuring all necessary referrals and documentation are provided.
Keep the work environment clean and well-organized.
Maintain strict confidentiality of patient information at all times.
Ensure patients see nursing staff prior to discharge.
Help manage and de-escalate patients with behavioral concerns.
Serve as a positive role model for patients and peers.
Take on additional responsibilities as assigned by your supervisor.
Qualifications:
CASAC-T certification (CASAC-T only)
High school diploma or equivalent
Ability to work shifts: 7:00 AM - 3:00 PM or 3:00 PM - 11:00 PM (including weekends)
