We are seeking Health Services Coordinator to join our dedicated team. In this role, you will assist patients in overcoming treatment barriers and keeping them engaged in their recovery services. Your actions will reflect resilience and encourage hope in our patients' recovery journeys.

Shift Information:

• Day Shift: 7:00 AM to 7:30 PM, three days a week, including every other weekend.
• Location: 20 minutes from Lockport.

Key Responsibilities:

• Interact with patients in a courteous, empathetic, and supportive manner at all times.
• Communicate consistently with counselors and nursing staff about patient issues.
• Promote positive interactions between patients and staff.
• Provide patient transportation as needed throughout the facility.
• Ensure adherence to the program activity schedule by patients.
• Conduct regular checks on patient rooms for safety.
• Perform census checks every two hours (four times during your shift), including after meals.
• Observe patients for any signs of inconsistent behavior and report findings.
• Assist in de-escalating situations involving distressed patients.
• Monitor and manage patient linens and laundry.
• Replenish linen supplies as needed and ensure proper disposal of soiled linens.
• Collect dirty laundry for housekeeping services and return clean items to patients.
• Document and inspect belongings of new admissions for safety compliance.
• Carry out other duties assigned by supervisors.
• Serve as a role model for both patients and staff.

Requirements:

Minimum Qualifications:

• High school diploma or equivalent.
• One year of related experience.
