Qualifications for Community Health Nurse (RN):

• Current RN license in New York State
• Skilled nursing facility experience preferred
• Reliable transportation required

Community Health Nurse (RN) Job Overview:

As RN, you will deliver quality nursing care to residents requiring rehabilitation or long-term care. The role includes planning, implementing, and evaluating patient care strategies in collaboration with other healthcare professionals.

Responsibilities:

• Follow the Nurse Practice Act and facility protocols
• Develop care plans and monitor patient progress
• Oversee nursing staff and ensure adherence to policies
• Keep accurate medical records and utilize equipment safely
• Educate patients and families on care and treatment
• Maintain patient privacy and ensure compliance with regulations
• Work scheduled shifts, including weekends and holidays

Education & Experience:

• Graduate from a recognized nursing program
• At least one year of nursing experience in acute or long-term care settings (critical care experience preferred)
• Must have a valid RN license in New York
