Outreach Referral Care Coordinator - Healthcare

We are seeking a talented and dynamic Clinical Care Coordinator to join our team!

The Clinical Care Coordinator serves as a vital bridge between the facility, marketing, and its clientele, as well as referral sources. With a dual focus on both nurturing client relationships and ensuring a smooth, continuous flow of referral communication and follow-up.

SHIFT
Mon-Fri 8:00a-4:00p or 9:00-5:00p

DUTIES/RESPONSIBILITIES:

• Develop and maintain relationships with the existing referral sources, such as hospitals, medical professionals, therapists, and community organizations for the purpose of streamlining patient care.
• Responsible for ensuring that coordination of care between clients and referral sources are maintained as deemed appropriate by client.
• Act as the point of contact for referrers as it pertains to patient care.
• Coordinate and conduct regular check-ins with referral sources, for patient updates, addressing any concerns and gathering feedback.
• Maintain strong rapport with counselors and clientele.
• Follow up with client’s post-treatment to gather feedback, testimonials, and success stories and to ensure successful transition into their next level of care.
• Coordinate with the facility’s care and administrative teams to implement suggested improvements when applicable.
• Collaborate with the marketing team to share, develop, and implement strategies and improved systems monthly.
• Maintain accurate and updated records of all client and referral source interactions in the facility's EMR and CRM system.
• Attending discharge meetings to coordinate with referral sources.
• Assist with discharge planning and client discharge plan completion as needed.

REQUIRED SKILLS/ABILITIES:

• Excellent interpersonal and communication skills, with the ability to interact compassionately with individuals battling substance issues.
• Proficient in EMR, CRM software and Microsoft Office Suite.
• Strong organizational and multitasking abilities.
• Strong ability to manage multiple relationships with various agencies.
• Ability to maintain confidentiality and act with discretion and empathy.

EDUCATION AND EXPERIENCE:

• Bachelor's degree preferred.
• Experience in client relations, case management, marketing, or outreach, preferably within a healthcare or substance facility setting is required.
