RN Case Manager

Position Overview

As the RN Case Manager, you will be responsible for managing the nursing program within our long-term care facility. Your role will ensure that all nursing practices meet industry standards, align with the center's policies, and comply with applicable healthcare regulations and guidelines.

Key Responsibilities

• Supervise nursing staff and oversee their involvement in quality assurance and utilization review processes.
• Contribute to the identification of clinical risks and assist in developing strategies for risk reduction.
• Ensure physicians' orders are communicated effectively and carried out by the appropriate team members.
• Oversee the delegation of nursing and therapeutic tasks to the appropriate staff members.
• Conduct regular rounds to assess residents' health and behavior, ensuring proper documentation and care delivery.
• Participate in incident investigations and ensure timely reporting to nursing management.
• Ensure adherence to infection control practices and biohazard waste management procedures.

Requirements

• Must possess a current RN license with prior experience in long-term care or working with the elderly preferred.
• CPR certification required.
• A minimum of three years of nursing experience and one year of supervisory experience.
• Strong organizational skills and ability to lead a nursing team effectively.
