Case Manager (CASAC)

As a unit counselor, you will assist patients by removing barriers to treatment and helping them stay committed to addiction recovery programs. You will set an example of positive recovery behaviors, share coping strategies, and inspire hope as patients navigate their recovery journey.

Key Responsibilities:

• Manage caseloads efficiently and effectively.
• Provide individual and group counseling sessions on a weekly basis.
• Conduct clinical assessments and intakes as required.
• Interact with patients in a professional, caring, and supportive manner at all times.
• Collaborate with other counselors and nursing staff to coordinate patient care.
• Foster a positive and encouraging atmosphere for both patients and staff.
• Ensure patients adhere to the program's daily activity schedule.
• Teach patients daily living skills and ensure their healthy functioning.
• Monitor patient behavior for inconsistencies and report findings to appropriate staff, including in shift reports.
• Under the supervision of a master-level QHP, help facilitate monthly skills workshops focused on chemical dependency.
• Assist in the preparation and processing of incident reports, following agency guidelines.
• Contribute to weekly case conferences and case reviews.
• Assist with the clinical discharge process, ensuring patients receive necessary referrals and documents.
• Maintain a clean and organized work area.
• Ensure confidentiality of all patient information, in compliance with regulations.
• Ensure patients are seen by nursing staff upon discharge.
• Help de-escalate patients experiencing behavioral challenges.
• Serve as a positive role model for both patients and peers.
• Take on additional tasks as assigned by supervisors.

Qualifications:

• CASAC-T certification (CASAC-T only)
• High school diploma or GED
• Availability for shifts from 7:00 AM to 3:00 PM or 3:00 PM to 11:00 PM (including weekends)
