Job Title: Long-Term Care LPN

Main Responsibilities:

• Handle the admissions, discharges, and transfers of residents.
• Process medication requests with pharmacies and physicians.
• Administer medications according to established guidelines.
• Adhere to infection control measures, emergency protocols, and other facility regulations.
• Collaborate with the interdisciplinary team to achieve the best possible patient outcomes.
• Monitor for and report any unusual changes in residents’ health to the appropriate personnel as dictated by policy.

Qualifications Required:

• A current Licensed Practical Nurse (LPN) license in the state is necessary.
• CPR certification is mandatory.
• At least two years of prior nursing experience is preferred.
