Care Coordinator (LPN) Responsibilities:

• Managing admissions, discharges, and transfers of residents
• Coordinating medication requisitions with pharmacies and healthcare providers
• Administering medications accurately
• Complying with infection control and other facility policies
• Collaborating with the interdisciplinary team to deliver the best resident outcomes
• Reporting any significant changes in residents' conditions promptly

Care Coordinator (LPN) Qualifications:

• Valid LPN license from the State
• CPR Certification required
• Preferably two years of nursing experience
